化脓性无菌性关节炎（Pyogenicsterilearthritis）、坏疽性脓皮病（pyodermagangrenosum）、痤疮（acne）综合征（简称PAPA综合征）是一种极为罕见的常染色体显性遗传的自身炎症性疾病。本文系一例孕妇并发PAPA综合征，无PSTPIP1突变。

病例资料
Thepatientwasa31-year-old,primiparouswomanwithPAPAsyndrome.Shehadthehistoryofvariousskinlesions(pyodermagangrenosumandseverecysticacne)andjointinflammationthatfirstappearedwhenshewas3yearsold.Herjointinflammationprogressed,andjointdeformitybecameprominentfromage15years.Whenshewas19yearsold,sheunderwentartificialjointreplacementofthethirdfingerofherlefthand.However,theartificialjointwasinfectedbymethicillin-resistantStaphylococcusaureus.Thus,theartificialjointwasremoved,andboneautotransplantationwasperformed.Herfacialacnebecameworseuptoage20years.Herskinlesionsandarthritisbecamestableattheageof26years.Atage30years,shewassuspectedtohaveanautoimmunedisorder.ConsideringherclinicalcourseandanelevatedserumlevelofIL-1β(21pg/mL;normal,10pg/mL),adiagnosisofPAPAsyndromewasmade.However,PSTPIP1genemutationassociatedwithfamilialPAPAsyndromewasnotobserved,andshehadnofamilyhistoryofthesyndrome.Thereafter,shehadbeenfollowedforPAPAsyndromeatourinternalmedicinedivision.Sheeventuallybecamepregnantandwasreferredtoourobstetricsoutpatientwardat7weeksofgestation.Herheightwascmandbodyweightwas56kg(bodymassindex,20.8kg/m2).Herbloodpressureattheinitialvisitwas/75mmHg.Distalinterphalangealjointdeformitiesandrestrictionsofmovementswereobservedonthefirst,second,third,andfourthfingersofbothhands.Furthermore,rigiditywaspresentintheinterphalangealjointofthesecondfingerofherleftfoot.Shealsohadskinpigmentationonthelowerextremitiesandbuttocks(Fig.1).
患者，女，31岁。初产，PAPA综合征。病史：多种皮肤病变（坏疽性脓皮病和严重的囊肿性痤疮）和关节炎症（患者3岁时出现）。在15岁时，患者关节炎症进展，关节畸形变得明显。19岁时，患者行左手中指人工关节置换术。但是人工关节受耐甲氧西林金黄色葡萄球菌感染。因而移除人工关节，并进行自体骨移植。20岁时，患者面部痤疮加重。26岁时，患者皮肤病变和关节炎稳定。30岁时，怀疑有自身免疫性疾病。考虑到患者的临床病程，血清IL-1β水平升高（21pg/mL；正常水平＜10pg/mL），诊断为PAPA综合征。不过未观察到与家族性PAPA相关的PSTPIP1基因突变，患者家族中没有此类病史。之后，在内科随诊观察PAPA。后来，患者怀孕第7周接入妇产科。患者身高cm，体重56kg（体重指数20.8kg/m2）。最初入院时血压/75mmHg。双手的食指、中指、第三和第四根手指均观察到远端指间关节畸形。患者左脚第二指指间关节僵硬，双下肢和臀部有皮肤色素沉着（图1）。
